Multi-Dimensional Validation of the Integration of Syntactic and Semantic Distance Measures for Clustering Fibromyalgia Patients in the Rheumatic Monitor Big Data Study
I am pleased to share our first hashtag#publication for 2024!
In this study, we describe a novel comprehensive multi-dimensional validation methodology for clustering patients, integrating both syntactic (based on data’s quantitative attributes) and semantic (based on meaning) distance measures. The novel methodology is termed Clustering, Distance measures, and Iterative Statistical and Semantic Validation (CDI-SSV). The CDI phase serves as the initial step, involving the evaluation of cluster quality, the impact of different starting seeds, and the consistency of clusters across various algorithms and pre-defined values of k. Within- and between-consistency checks, along with evaluations of internal semantic consistency, are performed to assess the optimal algorithm and values of k. In the subsequent SSV phase, an external semantic analysis of the results is conducted, with a particular focus on the clinical context, thus enhancing the validation process. Finally, machine learning techniques are employed to validate the results, and their interpretation is facilitated by SHAP (Shapley additive explanations) values.
We employed the CDI-SSV methodology in clustering and classifying fibromyalgia patients, demonstrating its potential applicability beyond fibromyalgia to other medical domains. The identification of distinct profiles within fibromyalgia patients allows for a more targeted and personalized approach to diagnosis, management, and prognosis. Ultimately, this work contributes to the evolving field of personalized medicine, offering data-driven insights and evidence-based practices that can transform patient care.
This research is part of the Rheumatic Monitor study, which focuses on advancing personalized medicine by identifying patterns that predict the severity of rheumatic diseases and treatment response. In the Rheumatic Monitor study, we developed a mobile application for iPhone and Android operating systems that collects baseline and dynamic questionnaires and includes an option to report on pain attacks and visualize pain reports and a large longitudinal EMR-based repository of 52,000 rheumatic disease patients.. More about the Rheumatic Monitor study and the application can be found on the research website: https://lnkd.in/di2SVP4C.

Pleased to share our new publication "Body Mass Index and Caries: Machine Learning and Statistical Analytics of the Dental, Oral, Medical Epidemiological (DOME) Nationwide Big Data Study" published in Metabolites (IF 5.58).
In this paper, we explored the association between BMI and caries using a large-scale, structured and comprehensive database of the DOME repository among a nationwide representative sample and used novel statistical and machine-learning approaches, which have not been carried out before. The DOME is a comprehensive large nationwide electronic record-based big data study, that measures dental-oral-systemic interactions using knowledge in dentistry, medicine, biostatistics, and computer science. As the PI of the DOME, I would like to thank my co-authors Ofir Ben-Assuli Gaya Geva Shlomit Siri Ori Bar for their contribution!

The Triangle of Nonalcoholic Fatty Liver Disease, Metabolic Dysfunction, and Periodontitis: Analysis of the Dental, Oral, Medical and Epidemiological (DOME) Records-Based Nationwide Research
Delighted to share our new publication "The Triangle of Nonalcoholic Fatty Liver Disease, Metabolic Dysfunction, and Periodontitis: Analysis of the Dental, Oral, Medical and Epidemiological (DOME) Records-Based Nationwide Research " published in Metabolites (IF 5.58).
The DOME is a comprehensive large nationwide electronic record-based big data study, that measures dental-oral-systemic interactions. This specific research explored the associations between Nonalcoholic Fatty Liver Disease (NAFLD) and dental morbidities in the context of Metabolic Syndrome, and periodontitis had been studied in the context of other dental and oral conditions that might exist concomitantly in an individual. While most studies focus on the association between periodontitis and diabetes and cardiovascular diseases, the associations with NAFLD were less studied.
As the PI of the DOME study, I am very delighted for every contribution of the DOME to the understanding of dental-systemic interactions and for the unique collaborative effort that uses knowledge in dentistry, medicine, biostatistics, and bio information. Thank you, co-authors, and in particular to Doron R., a fifth-year DMD student which did the study in fulfillment of his DMD thesis.

Tomorrow I will present the "RheumaticMonitor" study, as part of a conference organized by the “Inbar Association for Rheumatic, Autoimmune and Inflammatory Diseases” and the “Asaf Association for Fibromyalgia and Chronic Fatigue Syndrome”. We will hold a zoom meeting tomorrow with members of the associations for recruitment for research. As part of the RheumaticMonitor study, we developed a unique application for screening and monitoring patients with rheumatological diseases. The research is a collaboration that I lead as the lead in Hadassah Medical Center as a PI, together with Prof. Yuval Shahar and Erez Shalom, PhD. from Ben-Gurion University of the Negev . The research is funded by the Israeli Ministry of Science and Technology. The app collects baseline and ongoing information that is used for the analysis of the big data that accumulates over time from the patients, patients in order to discover predictive patterns for exacerbation, remission, and response to treatment.

The Painscreen – A New Tool for Screening, Monitoring and Analyzing Big Data of Pain Patients
The Hebrew University of Jerusalem, Israel
Background: The etiology and risk factors for progression of many chronic pain conditions (CPC) are still unknown. Moreover, real-time patient`s monitoring remains a challenge.
Aim: To develop a new digital tool for monitoring and analyzing big data of CPC patients in order to discover predictive patterns for :exacerbation ,remission, and response to treatment.
Methods: We developed the Painscreen, a new web application that works on mobile and stationary devices, in order to collect data and monitor patients continuously.
Results: The framework creates and stores answers to baseline and daily questionnaires and sensor data (such as :sleep patterns, steps) .Painscreen supports application, to the integrated longitudinal data, of big temporal data mining methods to discover predictive patterns, and present analysis results to the doctor.
Conclusions: The painscreen will lead to a more comprehensive understanding of CPC from a big data base, and will lead to better personalized care of CPC patients
